General
Summary: The Triage Nurse
appropriately triages patient symptoms both in the clinic and on the
phone. This includes nursing assessment, problem identification,
planning, reporting of complex clinical data to the appropriate cardiologist
and implementation of the patients plan of care as directed by the
cardiologist. This nurse will manage transition of care from acute
setting to home and support Care Team duties as assigned. Essential Functions and
Responsibilities: 1. Educates patients about their cardiovascular condition
and explains treatment options as directed by the cardiologist.2. Teaches patients what they can do to prevent the
worsening of cardiovascular conditions, the importance of self-monitoring and
provide appropriate resources.3. Triages patients physical symptoms /
arrhythmias appropriately and consults with cardiologists per protocol.4. Participates in Transition of Care Management to
ensure discharge medications and instructions are understood by patient and/or
family members. Updates medication and problem list in the Electronic Health
Record and ensures follow-up with cardiologists.5. Reviews physician tasks and clinical worklist(s)
as assigned.6. Reviews daily lab/test results, including INR
results when applicable and ensures appropriate management of results per MD.7. Assist the Clinical Nurse Director on matters
relating to nursing practices and participates in development of policies and
protocols for patient care.8. Maintains patient confidentiality per HIPPA.9. Assists in maintaining safety equipment / emergency
cart within the clinic.10. Maintains and helps to ensure that all
documentation is in a state of audit-readiness.11. Promotes ongoing communication and collaboration
with the clinical manager, physicians, and appropriate staff to ensure patient
safety and adherence to clinic protocols.12. Takes initiative to identify and provide
solutions to work-related problems, continuously seeking ways to improve
services.13. Occasionally covers for other staff as deemed
necessary both inter and intra departmentally.14. Updates Clinical Manager of changes affecting
their schedule.15. Professional representative within the clinic
and the community.16. Participates in clinic meetings as required.17. Participates in special projects as assigned.18. Attends continuing educational seminars to maintain
required licensure.19. Demonstrates positive initiative and cooperation
between departments.20. Other duties as assigned. QUALIFICATIONS:1. Excellent written and oral modes of
interpersonal communication.2. Maintains CPR and ACLS certification.3. Current CA Registered Nurse4. Minimum of two year cardiovascular clinic
experience and/or hospital nursing experience.5. Two years critical care hospital experience
preferred.
6. Two years case management experience in the
hospital or clinic setting preferred.
