work experience with heart failure patient population. Ability to work and communicate effectively
in a team environment. The RN -
Staff/Clinical Nurse will assist with delivering high quality, efficient
medical care to patients in the Heart Failure Clinical Excellence Program
within the Temple Heart and Vascular Institute. The RN will work with complex
heart failure patients and will support both the physicians and advance
practitioners with patient care management.
Responsibilities include: providing and/or coordinating timely direct and
indirect patient care services to include assessments, interventions,
referrals, documentation, telephone triage and pre-cert registration processes.
The
incumbent is also responsible for providing ongoing health education and
counseling activities; prompt abnormal testing reporting and follow-up; and
maintaining follow-up logs for record keeping purposes. The incumbent will travel
to satellite locations such as Oaks, Fort Washington and Fox Chase Cancer
Center. Performs other duties as assigned.Required Education and Experience:
Professional licensure as a Registered Nurse in the Commonwealth of PA.
Experience providing care management to complex heart failure patients.Required Skills and
