problem identification (nursing diagnosis), care planning (including desired
outcome determination), implementation and evaluation in nursing care in a
patient and family centered manner in accordance with licensure scope of
practice, regulatory standards, and established hospital policies, procedures

as: opiates, benzodiazepines, cocaine, bath salts. Discharge planning responsibilities
for timely preparation, thorough documentation and execution of patient discharge
management and planning beginning at time of admission including oversight of
multidisciplinary team timely documentation. Timely and thorough documentation
in a variety of manners including assessments, individual and group notes,
collaboration, collateral contact with internal and external stakeholders to
the patient. Instruct and run groups on a variety of topics and follow-up with
appropriate documentation according to guidance and prescribed deadlines and
educational goals. Individual counseling for an individual including a broader
picture of clients family and significant other when applicable and take part
