Senior Care Partners, a
private non-profit PACE (Program of All-Inclusive Care for the Elderly)
organization with the mission of providing comprehensive care for the frail
elderly population in order to allow them to remain safely in their home for as
long as possible with a focus on autonomy and independence. Our program
coordinates and provides care for the at-risk frail elderly population of our
community through an integrated team of health care professionals all under one
roof. This team meets daily to assess the complex health and social
needs of our participants and to make any necessary adjustments to the
personalized care plan for each individual. Our in-house day center, medical
clinic, and therapy rooms make it highly convenient for our participants to
receive the socialization they need as well as receive comprehensive medical
care under one roof. The Clinical Care RN provides direct nursing
care to participants in both a clinic setting and in our participants homes.
This role must utilize excellent critical thinking skills with the ability to
remain independent in the application of advanced nursing knowledge and skills
and the ability to manage complex clinical situations and help direct both
licensed and non-licensed clinic team members including but not limited to:
Care, support, and collaborate with the care
team including the participant and their family/caregivers using a
participant-centered care model.
Triage telephone
calls and on-site with critical thinking.
Monitors acute and
chronic medical conditions following the participants goals of care.
Performs wound care
and treatments as prescribed in their plan of care.
Administers
medications and immunizations and other care as prescribed and according
to patient care standards, identifies emergency situations and
independently initiates advanced therapy.
Performs
assessments and actively participates in care plans.
Makes acute home
visits as needed on a regular basis to provide care to participants or
give treatments (i.e., wound care, lab draws, nurse assessments, etc.).
Documents in the participants
medical record in our EMR TruChart.
Minimum of one year of
experience working with the frail, elderly, or long-term care population.
Minimum of one-year home care
experience required.
Minimum of one-year wound care
experience or CWCN preferred.
