JOB SUMMARYThe Licensed Practical Nurse (LPN) is responsible for the delivery of quality, long-term care to geriatric and disabled individuals as specified in individualized care plans; promotion of comfort, maintenance of functional abilities and respect for residents dignity, along with supervision of care provided by CNAs and support staff. REQUIRED KNOWLEDGE, SKILLS & ABILITIESEducation/Experience:   Current State of Montana LPN license.  Two to three years nursing experience in long-term care setting preferred. Technical Skills:  1.      Strong analytical and organization skills.2.      Ability to execute multiple tasks with minimal supervision.3.      Ability to handle stressful situations competently, effectively and courteously.4.      Management and leadership skills.5.      Ability to assist in lifting, transferring and positioning patients; agility to respond quickly to unexpected resident safety needs and medical emergencies.6.      Basic to intermediate computer skills. Other qualifications/certificate/license:  Current CPR certification. Behavioral Competencies:   Supports PMCs Code of Conduct and the organizations Mission, Vision and Values.  Maintains security and confidentiality of all information at all times.  Demonstrates the ability to function successfully in a team environment.  Exhibits courteous, compassionate and respectful treatment of all customers.  Displays a positive attitude, flexibility and ability to adapt to new and changing situations.  Participates in identifying potential problems or situations and suggests solutions. DECISION MAKING LATITUDEEmployee works independently, refers questions to Registered/Charge Nurse or Director of Nursing. JOB RESPONSIBILITIES1.      Administers, supervises and assists in overall resident care.   2.      Operates and maintains equipment to departmental standards3.      Performs safety and infection control procedures to ensure a safe and clean environment for the residents and staff. 4.      Complies with and maintains regulatory compliance requirements as defined for the nursing care on the unit and according to facility standards.5.      Completes and demonstrates competencies in required yearly unit competencies and education requirements as defined by the facility, and Director of Nursing.6.      Performs a comprehensive assessment of patient on admission and assessment or reassessment at regular intervals appropriate for the condition of the patient or as ordered by the provider7.      Rounds to observe residents conditions, assesses findings, plans nursing care, answers residents call light and records appropriate information into residents chart.8.      Administers medications, treatment, provider specific care to patients in compliance with facility and nursing policies/procedures9.      Makes arrangements for resident to see his/her provider(s) and notifies family member(s)/responsible party(ies).10.     Notifies provider(s), Resident Care Coordinator, Director of Nursing and other appropriate individuals when a change in the residents condition occurs.11.     Provides supervision to C.N.A. staff.12.     Completes resident clinical records including care plans, monthly, weekly and daily charting, medication and administration record, requests for laboratory and Imaging procedures and requests for pharmaceutical items.   13.     Notifies appropriate individuals of changes in residents condition to allow for decision-making in ongoing plan of care.14.     Other duties as assigned. Disclaimer: Not all inclusive other duties as assigned.  Every effort has been made to make this position description as complete as possible.  However, it in no way states or implies that these are the only duties required.  The omission of specific statement of duties does not exclude them from the position if the work is similar, related, or is a logical assignment to the position. Schedule:  Shift work to include weekends and holidays.
