Job Title: Licensed Practical NurseDepartment: NursingReports To: Unit Manager, Charge Nurse, Supervisor, Position Purpose: The License Practical Nurse (LPN), under the supervision of a Registered Professional Nurse (RN), provides direct resident care as delegated, and assists the RN in the delivery of resident care utilizing the nursing process as appropriate to the geriatric resident. Participates in resident care services as directed by supervisor in accordance with in-service training; effectively documents all interventions/care provided in the medical record.Educational Requirements: Graduation from an accredited LPN program.Experience Required: One year working in a direct patient care area preferably in a long-term care setting.Physical Requirements: While preforming the duties of this job, the employee is regularly required to stand, walk, talk and hear, and use hands and arms to handle items, lift, feel, and reach. The employee is frequently required to sit, stoop, kneel, or crouch.Professional Licensure and Certification Required: Current, unencumbered New York State LPN license. Current CPR certification and IV certification.Interpersonal Relationships: Communicates effectively with residents, families, visitors, co-workers, physicians, and other employees.Essential Functions1. Adheres to rules of attendance as outlined and received on orientation to the facility.Functions in the role as staff nurse and provides direct care to meet the needs of the residents as needed/required.2. Participates in inspections (surveys) conducted by authorized agencies.3. Provides safe resident care while demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies.4. Maintains awareness of new updates/standards and incorporates them into his/her nursing practice throughout the continuum of care.5. Participates in quality assurance and improvement processes within the facility.6. Incorporates age-specific needs, provision of care, procedures, and treatments.7. Accurately completes routine tasks without the need of direct supervision.8. Effectively communicates pertinent resident information at change of shift and during transfer of residents.9. Accurately transcribes and administers medications according to the five rights of medication administration and documents the same.10. Accurately documents care provided, along with updating resident care plans with any changes in plan of care, as part of routine responsibilities of the LPN.11. Accurately performs treatments utilizing aseptic or clean technique. 12. Accurately administers prescribed enteral feedings.13. Demonstrates the mission, vision, and values of the facility through professional excellence.14. Completes resident care-related activities within scheduled shift time periods.15. Exhibits calm behavior and uses professional judgement while working in a team approach during emergency situations.16. Cooperates with reassignments made in the interest of resident care.17. Assists co-workers as needed and maintains positive interpersonal relationships with colleagues.18. Exhibits professional conduct and behavior through cooperative interactions with others and accepts personal responsibility for their own actions.19. Reports any change in condition of a resident to the appropriate supervisor.20. Reports any resident care issues to the appropriate supervisor.21. Effectively delegates responsibilities to the CNA staff on a daily basis.22. Reports for their assigned shift in a timely fashion.23. Adheres to the facility dress code.Staff Development Functions1. Attends departmental meetings as scheduled to assist in identifying and correcting problem areas, and/or improvement of services.2. Participates in the orientation of new staff members to the facility and ensures that appropriate education and mentoring is in place.3. Attends and participates in scheduled training and educational classes.4. Attends and completes all mandatory in-service programs.Safety and Infection Control1. Follows established protocol during fire drills and disaster training.2. Follows established universal precautions and infection control policies and procedures when performing daily tasks.3. Ensures that all work and resident care areas are maintained in a clean, safe, and sanitary manner.Resident Rights1. Maintains confidentiality of all pertinent resident care information.2. Knowledge of facility and state mandatory reporting practices regarding any suspected cases of abuse, mistreatment, or neglect.3. Report all grievances/complaints made by the resident to your supervisor immediately.4. Ensures that all residents are treated fairly with kindness, dignity, and respect.5. Provides nursing care in a manner that ensures all residents are treated fairly and with kindness, dignity and respect.
