Position Reports To: Charge Nurse/Supervisor
BASIC FUNCTION:
Under the
direction and supervision of the unit charge nurse, and shift supervisor,
provides the basic nursing care to residents.
ESSENTIAL FUNCTIONS:
1. Resident
Care Function:
a. Perform
routine nursing care which includes bathing, dressing, undressing, personal hygiene,
shaving, and keeping residents clean and dry.
b. Assist
residents with mobility, including transferring residents in/out of bed, wheelchairs, lift, etc.
c. Assist
in admission, discharge, transfer, and death procedures.
d. Assist
in supplying daily nourishment and fluids to residents (mealtime, snacks, fluids,
etc.). Document intake and output when
indicated.
e. Take
resident vital signs, including temperature, pulse and respiratory rate, and
blood pressure. Obtain
resident weights.
f. Assist
in pressure ulcer prevention by proper positioning, use of pressure relieving devices,
and preventative skin care.
g. Collect
specimens (urine, stool, sputum) as directed.
h. Assist
in keeping the resident environment clean and neat, including making the residents
bed, appropriate handling of hygiene items, and cleaning dressers, night stands
and closets.
i. Answer
resident call bells promptly. j. Plan and organize care so that
resident can attend activities and therapies as
scheduled.
k. Make regular rounds to all residents
and provide care as necessary.
l. Participate
in the resident plan of care conference when indicated.
m. Report
incidents/accidents promptly to charge nurse and/or shift supervisor.
n. Remain
on nursing unit, providing care until relieved by oncoming CNA; may be
required
to work after scheduled shift until relieved.
o. Be available to assist facility in
times of staffing shortages.
p. Participates in Preventative Nursing
Program after resident is discharged from
Restorative
Nursing Program.
2. Communication
Function:
a. Record
all pertinent data on the Nursing Assistant Daily Care Record and Daily Assignment
Sheet. Complete other nursing care forms as required.
b. Report
changes in the residents condition, resident request for medications, resident
complaints/concerns,
and the residents response to care interventions to the charge nurse.
c. Maintain
good intra-personal relationships with resident, family, visitors, and other personnel.
d. Be
polite and available to state survey team during surveys.
e. Participate
as an active team member in abuse prevention and reporting.
3. Environmental/Safety
Function:
a. Assist
in keeping the resident environment clean and neat, including making the residents
bed, appropriate handling of hygiene items, and cleaning dressers, night stands
and closets.
b. Keep
resident rooms and bathrooms free of safety hazards.
c. Report
broken or malfunctioning equipment to the charge nurse and remove same from use.
d. Keep
hallways and emergency exits free of clutter.
e. Use
good body mechanics in the provision of resident care.
f. Keep
clean and soiled utility rooms, linen closet, and storage areas clean and free
of clutter.
4. Personal
Development Function:
a. Attend
and participate in facility inservice programs (12 hours/year minimum).
b. Maintain
confidentiality of resident, personnel, and facility information.
ANCILLARY DUTIES:
1. Other
related duties as requested.
QUALIFICATIONS:
1. Must be on the Pennsylvania Registry as
a nursing assistant; or a graduate of an approved Nursing Assistant Training Program, or be eligible
to enter such a program, graduate, and pass a state approved competency
evaluation within one hundred twenty
(120) days from the date of employment.
2. Must participate in at least
twelve (12) hours of inservice education each year to include but
not
exclusive to mandatory inservices.
PHYSICAL REQUIREMENTS:
1. Must be able to move intermittently throughout the workday
(sitting, standing, bending, walking,
and lifting).
2. Must
be able to see and hear, or use prosthetics that will enable these senses to
function adequately
to ensure that the requirements of this position can be met.
3. Must be in good general health and demonstrate emotional
stability to cope with the mental and
emotional stress of the position.
4. Lifting
restrictions of no more than fifty (50) pounds.
WORKING CONDITIONS: 1. Works
